(a)
Securities Trading Application Form-Individual
(b) Two Specimen Signature Cards
(c) Securities Brokerage Agreement
- Complete all the above documents and sign your signature where required. Please note that the signature should be same as shown in the original passport.
- Required Documents for Opening an Online Trading Account: (Please sign your signature on all copies. Please note that all your signature must be same as shown on the original passport.)
3.1) Applicant:
• Copy of identity card / Passport *
• Copy of house registration (for Thai applicant only)
• Copy of Work Permit of client (if any).
• Copy of bankbook which show your bank account name/number.
3.2) Beneficiary / Controller / Related Persons (if any):
• Copy of identity card / Passport *
• Copy of house registration (for Thai only)
Remark * :
• Foreigner : A copy of your passport signed on each page (Certified signature must be the same as shown in the passport)
• Thai : A copy of your Identification card (ID) certified by your signature.
