   (1) Derivatives Trading Application Form - Individual
   (2) Two copies of the Specimen Signature Card.
   (3) Letter of Consent for Debit to the Bank Account (ATS) for Additional Margin Call Payment.
   (4) Letter of Consent for Debit to the Bank Account (ATS) [In case of additional collateral required by Margin Call per (3) and (4) is usable only via Bangkok Bank Plc, Siam Commercial Bank Plc.or Krung Thai Bank Plc.]
   (5) Derivatives Agent Appointment Agreement.
   (6) Risk Disclosure Statement on Derivatives Trading.
   (7) Attachment to the Derivatives Agent Appointment Agreement.
   (8) Power of Attorney (if any).
   (9) Two copies of the Specimen Signature Card for Attorney-in-fact (if any).
   
   
2. Complete all the above documents and sign your signature where required.
   
   
3. Documents required for Derivatives account opening (certified by your signature)
   
   3.1) Applicant:
   •  A copy of ID card or passport (for foreign investors)
   •  A copy of House Registration (for Thai applicant only)
   •  A copy of financial statement, for example
        -   A copy of bank statement dated back at least 3 months, along with the
             front page of your bank book showing your name and bank account
             number, certified by your signature
        -  The latest salary pay slip
   •  A Copy of TFEX Training Certificate
   
   3.2) Beneficiary / Controller / Related Persons (if any):
   •  A copy of ID card or passport (for foreign investors)

   3.3) Grantee of Power of Attorney (if any)
   •  A copy of ID card or passport (for foreign investors)
   •  A copy of House Registration (for Thai applicant only)
   

4. Duty Stamp
   
   •  THB 30 duty stamp for Derivatives Agent Appointment Agreement
   
   •  THB 30 duty stamp for each grantee of Power of Attorney (if any)
   

5. Please Send all documents to:
