1. Click here to Download & print the Account Application Forms which are comprising of:-
   
   (a) Individual Securities Trading Application Form
   (b) Securities Brokerage Agreement
   (c) Two Specimen Signature Cards
   
   
2. Complete all the above documents and sign your signature where required. Please note that the signature should be similar as shown in the original passport.
   
   
3. Attach a copy of your passport, certified the true copy by signing your signature on every page.
   
   
4. Forward the above document in item 1 and 3 along with 30 baht duty stamp to the below address:-

5. Once received, and subject to acceptance, we will advise you your permanent login name (account number) and password to the address given in the application.
   
   
6. For your own benefit, foreign investors who would like Seamico to credit the dividends, for which you are eligible, to your stock trading account are required to specify "Address for Notification of Benefits/Interest" in the Application Form <Item 1(a) Page2> to be Seamico Address only.
