   (5) Derivatives Agent Appointment Agreement.
   (6) Risk Disclosure Statement on Derivatives Trading.
   (7) Attachment to the Derivatives Agent Appointment Agreement.
   (8) Power of attorney (供选择)

2. 填写以上表格,并且在签名处签名.
   
   
3. 其他的申请文件 (全部文件都必须签名确认)
   
   3.1) 申请人 :
   •  护照复印件1份,并签名
   
   •  个人资产证明,例如
   
   • 至少开户3个月的银行储蓄本,资金流动以及储蓄本首页复印件一份,以确认您的姓名和银行帐号.
   • 上个月的工资单
   
   •  接受过TFEX培训的证明复印件1份
   
   3.2) 收益人/管理人/联系人(供选择):
   •  个人身份证或护照复印件 (外国人)

   3.3) 法律担保人 (如果需要)
   •  个人身份证或护照复印件 (外国人)
   •  住房注册号 (泰国申请人)
   
   

4. 手续费

• 法律担保人费用30泰铢 (供选择)
